This form is for, if you don’t want to allow to release your personal information, or in the case that you want to release someone of your choose of your information. The person you choose will be able to get information on your case file or reading such as the status of your application.
Sadly to say, I’ve had people call and ask what did you say to husband/wife or what came up in my Son’s reading. Then as I don’t say anything about any client’s reading, the next thing I do is to call them and let them know, so and so has called asking to get information on your reading, only to get ” oh sure that’s my Mom ,you can give her the information. This is a way of covering my butt.
